Medal bar questions…..

This is the first time I’ve ever seen in person, touched, or owned one of these medal bars. I just had a few questions. Of the pictures I’ve seen of these online, most of them, kind of hang in the middle on the front of the bar. Where as this one clearly hangs from the bottom. The story i heard about these, was that it was up to the soldier to go to a jeweler to have this done, via private purchase. Is there truth to that? Is this a common backing for one of these? Also, I was wondering were these typically just worn like this during parades, ralliew, and or honoring soldiers, etc.? Thanks.
