pre-war Polish passport issuing clauses & costs?

Good morning!
I am trying to find an official guide that ministry of interior and consulates used when issuing passports and the cost-clauses for this, for example:

bezpłatny was under clause 8
refugees was under clause 11

what was clause 16?

This seems to be an odd question…
