I’m attempting to translate this (hopefully legit) WW2 Japanese Dog Tag. I read Nick’s Evolution of army dog tags and was able to get this far with it.

Not sure if what I have notated in red is correct and would appreciate any assistance on the kanji marked with a ?. …or corrections to what I have translated incorrectly.
